I hereby REQUEST the REGISTRATION as a collaborator of the SALAM GAZA ASSOCIATION from the date indicated, accepting the terms reflected in the Statutes of the Association.

For the purposes of the provisions of Law 15/1999 of December 13, 1999 on the Protection of Personal Data and other implementing regulations, the undersigned authorizes the use of his/her personal data contained in the documents submitted and their processing in a file owned by the Association, for the sole purpose of the resolution of the file, as well as to manage any aspect related to its relationship with the Association. I am aware of the rights of access, rectification, cancellation and opposition of the data contained in the aforementioned file, and may exercise these rights by sending a letter to the Association at the address indicated at the beginning of this application, duly signed, including your full name, address and a photocopy of your Identity Card (ID) or any other valid document.
